High School Registration in Two Easy Steps

1) Complete the High School Registration Packet

• Open & read the Registration Packet (Interactive PDF).
• Type information on all pages of the form except on the locked signature lines.
• Click the submit button in the upper right hand corner of the form and then print two copies and sign the forms in the places indicated.
• Keep one copy and bring a signed copy along with your registration fee to practice on 9/22/08 if you did not mail as requested.

• If your son has not already completed the new physical requirements for 2008-2009 print the form, schedule the physical and submit to the club secretary by 9/29/08.
• The good news is that this will satisfy the physical requirments for the spring too!

2) Join US Lacrosse - Youth Membership

• You will need this number to complete the registration packet
• Remember to print and submit a copy of your registration for verification
• The good news is that this annual membership will satisfy the requirments for those wishing to play in spring and summer too!
